Add the ability to generate a CSR with an Alternate name

In the Netreo UI, there is the ability to generate a CSR and then install a cert. However, there is no field or option when generating the CSR to specify an Alternate name.

Alternate names are becoming more and more and more common and it would be helpful if that option was available.
